Transition-Age Services

Independent Living Program services at Carolina Outreach provide young persons ages 15-21 the knowledge and means to transition successfully into adulthood. The Independent Living Program utilizes a continuum of community support, independent living skills classes, vocational and educational opportunities, need-based financial and housing supports. This support is provided using a team treatment model consisting of two Licensed Clinical Social Workers and a Peer Support Specialist. Participating youth work with the ILP Team to achieve independent living goals – while avoiding negative outcomes such as homelessness, unemployment, substance abuse, or incarceration.
