TCM – Targeted Case Management services at Carolina Outreach assist children (under age 21) who have a serious emotional disturbance, mental illness, or a substance abuse disorder and who are in a residential setting and need coordination of services (including medical, behavioral, and social) to transition to a less restrictive level of care. TCM is individualized, person-centered, empowering, comprehensive, strengths-based, and outcome-focused. The functions of case management include the following:
