Child Services - Targeted Case Management

TCM – Targeted Case Management services at Carolina Outreach assist children (under age 21) who have a serious emotional disturbance, mental illness, or a substance abuse disorder and who are in a residential setting and need coordination of services (including medical, behavioral, and social) to transition to a less restrictive level of care. TCM is individualized, person-centered, empowering, comprehensive, strengths-based, and outcome-focused. The functions of case management include the following:

  • Case Management / Assessment
  • Person-Centered Planning
  • Referral / Linkage
  • Monitoring / Follow-up

Expected outcomes for children receiving TCM services from Carolina Outreach are that they and their families show progress towards safety, health, and recovery, which includes the following:

  • Client develops a single, comprehensive PCP that addresses all service and support needs
  • Client is linked to a physician and is receiving good care
  • Client is linked to natural supports as available
  • Client transitions from a residential setting to a lower level of care
  • Client has fewer crisis episodes that require emergency interventions
  • Client becomes increasingly independent in managing his/her own care

Services are provided to clients in a variety of settings, including homes, offices, schools, courts, secure juvenile detention centers and jails, homeless shelters, libraries, street locations, and other locales in the community.
